The center management is aware of the implementation of this self-evaluation process

For each self-evaluation by Committee, a clinical manager and an administrative manager have been appointed to manage the self-evaluation.

You have ensured that the same organization whose evaluation you are going to register has not completed another previous evaluation on this website within the last three months.

You declare under your responsibility and for the relevant legal purposes that the completion of the evaluation and the request and download of the evaluation reports are carried out on behalf of the organization you have registered and not for the use of the information by third parties.
